Your SOAP Note

After you click Generate Note, Nesso will transcribe the audio and create a structured SOAP note with four clear sections: Subjective, Objective, Assessment, and Plan.

Viewing your note

  1. From your Dashboard, click a session to open Session Details.
  2. You can also click on All Past Sessions to view the list of all the notes you've written.

Editing your note

Just click inside any section to make changes. And don’t worry—your edits save automatically, so you won’t have to manage anything manually.

Customizing note settings

You can adjust how your note is written anytime:

  • Format: Bulleted or Narrative
  • Length: Concise or Standard
  • Identifier: Client or Patient

When you change a setting, Nesso will regenerate the note and replace the current content—including any manual edits you’ve made—so be sure you’re ready for a refreshed version.

Copying your note

Click Copy All to copy the entire note to your clipboard, then paste it into your EHR. If you prefer, you can also copy individual sections.

Marking a note as Done

Use Mark as Done to keep track of which sessions still need attention. If you want to revisit something later, you can always undo it before making further edits.

Changing the treatment approach

You can update the treatment approach from the Note tab. Once you do, Nesso will regenerate the note so everything stays aligned with your updated approach.

Error messages

  • "Unable to Generate SOAP Note": Nesso couldn’t capture usable audio. Please double-check that your microphone is working.
  • "No Transcript Available": The transcript was deleted, so the note can’t be regenerated.
