Your SOAP Note

After you click Generate Note, Nesso transcribes the audio and generates a structured SOAP note with four sections: Subjective, Objective, Assessment, and Plan.

Viewing your note

  1. From your Dashboard, click a session to open Session Details.
  2. The Note tab shows your SOAP note. The treatment approach (e.g., CBT) appears at the top.

Editing your note

Click inside any section to make changes. Edits save automatically.

Customizing note parameters

Adjust how your note is written:

  • Format: Bulleted (default) or Narrative
  • Length: Concise (default) or Standard
  • Identifier: Client (default), Patient, or Name

Changing a parameter regenerates the note, replacing current content including any manual edits.

Copying your note

Click Copy All to copy the entire note to your clipboard, then paste into your EHR. You can also copy individual sections.

Marking a note as Done

Click Mark as Done to track which sessions still need attention. You can undo this if you need to make further edits.

Changing the treatment approach

Update the treatment approach from the Note tab. This will regenerate the note.

Error messages

  • "Unable to Generate SOAP Note": No usable audio was captured. Check that your microphone was working.
  • "No Transcript Available": The transcript was deleted, so the note can’t be regenerated.
